For a story line I am doing it is necessary that my sim (Brent) look exactly like his great grandpa, so I want to use sim surgery to accomplish this.

Great Grandpa Brett was an elder when he died. I saved his gravestone, so he does show up in the sim pool for the surgery option.

But since he WAS an elder, does that mean that when Brent gets his surgery accomplished that he will look like an elder when he's an adult? Or does it automatically pull Grandad's adult genetics instead?

I'm pretty sure that your Sim will look like an elder but act like an adult. Sim Surgery uses the person's features regardless of age so you might want to clone the grandfather in Body Shop and turn him into an adult or whatever age you need and then use Sim Surgery. It's just a matter of finding his package file and uploading it to Body Shop.

It depends. Somtimes the game does not delete the Information that was needed to display a Sim as an adult, when it updates the Lifestage.

You can check this by opening the character file, and check how many property Set Files are included there. (one for each lifestage).

When you clone a Sim, it will of cours fist get the exact same appearance as the archetype sim (including the lifestage), but you could copy just the skineton, face and eyes. Which would not change the lifestage of a Sim!
