| The best approach would be creating a new Sim in the CAS that has the new Skin, the select that new Sim as archetype in Sim Surgery, and copy it's skin.
The less clean way (because it has the potential of Errors, when you do have custom content installed, that is not as clean as SimPe wants it) is, to add the Downloads Folder to your FileTable (in Extra Preferences, don't forget to reload the FileTable afterwards), and start Surgery again. That way, it will list the custom Skintones in the Skin Selection Box, and you can choose it directly, without having to create a new Sim. |

| Did you just install it from a sims2pack? If so, you need to run the game to finish the installation of it.
If not, did the skin go into saved sims? |

| the same thing happened to me i think what fixed it was making it recursive. also i added the folder before i loaded the neighborhood browser. try both those things. |

| If its in a subfolder, you'll need to make it recursive so it will scan the subfolders. |
